Do you take my insurance?

Cascade Physical Therapy & Sports Medicine LLC has chosen to remain out-of-network with all insurance companies, but you can still use your insurance benefits!CPTSM is a cash based model, meaning you pay for services up front and we don't charge your insurance company. Upon request I am happy to provide you with a super bill which you can submit to your insurance to have any out-of-network benefits applied and receive any reimbursement you may be eligible for, or have the out-of-pocket costs applied to your deductible.
I am also happy to discuss payment plans, multiple visit packages, and other discounts. Please don't let the out of pocket cost be your reason for not reaching out for help!

Why would I pay to see you instead of going to a PT clinic that takes my insurance?

When the physical therapist enters into a contract with your insurance, the insurance company dictates the treatment that you receive. In a cash based model, the physical therapist is entering into a contract with YOU to provide treatment in a way that will help you meet your desired goals of care most efficiently. Many clinics utilize support staff such at PT assistants, or rehab aids, and you may not see the same provider each time. At CPTSM your entire appointment with be 1:1 with Megan, a Doctor of Physical Therapy, every single time. Because of this, it is also likely that you will not need to complete as many visits to reach the same outcome since we can provide more focused care on you, when you need it.

Do I need a referral to see you?

No. Oregon is a direct access state. This means, you may have an evaluation and treatment for PT without a script/referral from your physician.

PLEASE NOTE: If you plan to submit to your insurance for reimbursement, your insurance company may require a script prior to beginning PT. Please inquire with your insurance company for more information.
